The Home and Community Based Services Waiver at the Jewish Board of Family & Children’s Services is seeking to hire full-time Sr. Case Managers/Individualized Care Coordinators (ICC’s) to provide intensive mental health case management services to children at risk of hospitalization and their families. Job duties include case management services, clinically focused short term interventions, supervision of per diem staff, and crisis response. This position will work primarily in the homes and community-based organizations.
Qualified candidates should possess a Master’s degree from an accredited college in a related field or a NYS Teacher’s Certificate and at least two (2) years experience providing direct services for children or a Bachelor’s degree along with four (4) years experience providing direct services to children. Candidates must have a car and valid driver’s license.
